During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 4/14/2025 and completed on 4/14/2025. The on-site inspection began at 11:50 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 19. The inspector reviewed 4 children?s records, 2 caregivers, and 2 household member(s) records on-site. This inspection included document review, a tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/18/2025. A POC submitted after this date will not appear on the public website.
Violations
5Child#1 is missing documentation of a physical examination.
The immunization for Child #1 was not signed by a licensed physician, the physician's designee, or an official of a local health department.
For assistants documentation that two or more references as to character and reputation as well as competency were checked before employment.
Caregiver #2, who had been employed for over five months, is missing one reference.
On top of a shelf next to the children's bathroom were several bottles of diaper cream that were accessible to the children in care.
There was no documentation of immunizations for Child #3 and Child #4, who were in care during the inspection and had been in care for 29 days.
