Inspector Notes

An unannounced monitoring inspection was conducted on 11/02/2023 from 9:30 a.m. to 10:40 p.m. There were six children present, ranging in ages from two to four years old with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Five child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records, the provider failed to obtain documentation of a physical examination prior to the child?s attendance or within 30 days of attendance.
