Inspector Notes
A monitoring inspection was conducted on 8/5/22. There were 4 children (11 points) present, with 2 caregivers supervising. The children were observed playing with play dough, listening to music and eating snack. 2 children's records, 3 caregiver records, emergency drill records and attendance was reviewed. There were no children with diagnosed food allergies or medications on site.

Violations
2Evidence:
1. The provider stated that caregiver # 1 at the family day home starting on 6/6/22. Caregiver # 1 worked for approximately 3 weeks.
2. Caregiver # 1's record indicated that she resided in Arizona and Texas within the last 5 years.
3. Caregiver # 1's sex offender searches for Arizona and Texas were not completed prior to the first day of employment.
4. The criminal record checks for Caregiver # 1 for the states of Arizona and Texas were not obtained prior to the first day of employment, and were unavailable to this date.
Evidence:
1. The provider stated that Caregiver # 1's first day of employment was 6/6/22.
2. The provider stated that she had completed orientation with Caregiver # 1 but written documentation was not maintained.
