Inspector Notes

A monitoring inspection was conducted on 8/5/22. There were 4 children (11 points) present, with 2 caregivers supervising. The children were observed playing with play dough, listening to music and eating snack. 2 children's records, 3 caregiver records, emergency drill records and attendance was reviewed. There were no children with diagnosed food allergies or medications on site.

Standard 22.1-289.035-B-4
Based on staff interview and record review, the family day home did not obtain results of an out of state criminal record check and sex offender search for an employee prior to the first day of employment.
Evidence:
1. The provider stated that caregiver # 1 at the family day home starting on 6/6/22. Caregiver # 1 worked for approximately 3 weeks.
2. Caregiver # 1's record indicated that she resided in Arizona and Texas within the last 5 years.
3. Caregiver # 1's sex offender searches for Arizona and Texas were not completed prior to the first day of employment.
4. The criminal record checks for Caregiver # 1 for the states of Arizona and Texas were not obtained prior to the first day of employment, and were unavailable to this date.
Plan of Correction: The background checks will be completed for future use. The sex offender searches will be obtained and all results will be sent to the licensing inspector.
Standard 8VAC20-800-200-C
Based on record review, the provider did not maintain documentation of a signed and dated orientation fo9r each caregiver.
Evidence:
1. The provider stated that Caregiver # 1's first day of employment was 6/6/22.
2. The provider stated that she had completed orientation with Caregiver # 1 but written documentation was not maintained.
Plan of Correction: Orientation paperwork will be competed and placed in file.
