Inspector Notes
A Subsidy Health and Safety inspection was initiated on 5/4/2022 and concluded on 5/5/2022 to review supplemental health and safety requirements. There were 114 children present with 23 staff supervising.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of 10 staff records and interview, the center did not ensure that 1 staff obtained current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment as required.
Evidence:
1. The record of staff #4 (DOH: 8/3/2021) did not contain documentation of a CPR certification.
2. Administration acknowledged that the staff was not certified.
