Inspector Notes

The licensing inspector completed an unannounced complaint inspection on February 8, 2023 in response to a complaint that was received by the Department on February 6, 2023 concerning programs. The investigation was initiated on February 8, 2023 and completed on February 14, 2023. On February 8, 2023, the inspector was on site from 1:00 pm to approximately 2:45 pm interviewing staff, conducting observations at the center, and reviewing documentation. Additional records were reviewed on February 14, 2023. The preponderance of the evidence gathered during the investigation does not support the allegation regarding programs, therefore the complaint is determined to be not valid. There were no violations cited.
