Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on November 4, 2024. The on-site inspection began at 9:50 am and ended at 2:05 pm. The inspector reviewed compliance in the areas listed above. There were 136 children present and 23 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

A tuberculosis screening was due for staff #8 was due 4 months ago. An updated tuberculosis screening has not been completed.
Plan of Correction: Staff will not start w/o calendar reminders for renewals.
Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Staff #4 completed 7 of the 16 hours required hours in 2023.
Plan of Correction: Staff are required to complete as a part of job requirements. Will not be able to continue until complete.
Standard 8VAC20-780-270-A
(Repeat Violation) Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

In an activity room in the school age building, there was a large tangle of electric cords visible and accessible to children.
Plan of Correction: Will remove and ensure locked away.
Standard 8VAC20-780-60-A
(Repeat Violation) Each center shall maintain and keep and keep at the center a separate record for each child enrolled that contains all of the required information.

1. The record for child #1 who was enrolled 2 months ago, did not contain a home phone number for each parent.
2. The record for child #2 was enrolled 1 month ago, did not contain a home phone number for both parents.
3. The record for child #3 who was enrolled 3 months ago, did not contain a home phone number for both parents.
4. The record for child #4 who was enrolled 3 months ago, did not contain a home phone number for both parents.
