During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 30, 2025. The on-site inspection began at 9:20am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 95 children present and 17 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included a tour of the facility, document review, observations, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice.

Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

The resilient surfacing around the play equipment on the preschool playground, had mulch that measured approximately 2 to 3 inches in depth.
Plan of Correction: Will shift/refresh mulch and invest in getting rubber mats to sustain proper levels.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #4, #5, and #6, enrolled for over 30 days, was missing a home phone number for parents and a work number for parents.

The records for child #7, #8, and #9, enrolled for more than 30 days, were missing a home phone number for parents.

The record for child #10, enrolled for nine days, was missing a home phone number for parents and the parent?s work number.
Plan of Correction: Will remind and correct families to completely fill out paperwork or include "n/a".
