An unannounced, on-site monitoring inspection was initiated on January 31 2025 and completed on January 31 2025. The on-site inspection began at 12:00pm and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 2 children present and 2 caregivers; the point total was 5 . The inspector reviewed 2 children?s records and 2 caregiver/household member(s) records on-site.

This inspection included a document review, a tour of the home, interviews and observation

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
