Inspector Notes
An unannounced renewal inspection was conducted on 05/19/2022. There were 3 children, ages 9 months- 2 years, totaling 10 points, one care giver and one household member present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 1 caregiver record, and 1 household member record were reviewed. The children were observed in free choice play. The inspector discussed the following: pets, nutrition and infant care. The inspector arrived for the inspection at 9:20 am and departed the family day home at 11:30 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations

1. The most recent findings from a search of the central registry for Caregiver 1 was dated 04/12/2016; there was not a 2021 follow-up search available for review during the inspection.
2. The most recent findings from a search of the central registry for Household Member 1 was dated 04/12/2016; there was not a 2021 follow-up search available for review during the inspection.
Evidence: There was an aboveground pool on the property. The ladder was in the pool. There was approximately 6 inches of water in the pool.
Evidence:
1. There was an opened can of interior wall paint on a table in the living room of the home. The paint was accessible to children.
2. The bathtub in the bathroom used by children was full of cans of interior wall paint. The paint was accessible to children.
3. There was an aerosol can of 409 in the dining room of the home. The 409 was accessible to children.
4. There were 2 spray bottles of fabric freshener under the fish tank. The fabric freshener was accessible to children.
Evidence:
1. There were 2 uncovered, unused electrical outlets in the living/dining room area of the home.
2. There was a surge protector by the fish tank. There were several uncovered, unused outlets on the surge protector that were accessible to children.
3. All of the children in care were under five years of age.
Evidence:
1. The record for Child 1 did not contain the following: immunization records for the child as required by the standards.
2. The record for Child 2 did not contain the following: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (contact 1 was a parent); immunization records for the child as required by the standards; and results of the health examination for the child as required by the standard.

The record for Child 3 did not contain the following: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (contact 1 was a parent); immunization records for the child as required by the standards; and results of the health examination for the child as required by the standard.

The record for Child 4 did not contain the following: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (contact 1 was a parent); immunization records for the child as required by the standards; and results of the health examination for the child as required by the standard.

Evidence: There was a white oval pill in a bowl on the provider?s dining room table that was accessible to children. The provider stated that it was her pain pill that she had not taken that morning.
