Inspector Notes

An unannounced on-site monitoring inspection was conducted on 12/19/2023. There were 6 children, ages 8 months-4 years, and 2 staff members present during the inspection at a total of 18 points. The inspector observed the following: snack, TV time and hand washing. A total of 5 children?s records and 2 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 3:15 PM and departed at 4:45 PM

Standard 8VAC20-800-180-A
Based on review of staff records, the provider failed to ensure that a repeat TB screening was completed every two years.

Evidence:
The provider and assistant most recent TB screening was dated 11/22/2021.
Plan of Correction: The provider and assistant have an appointment in the beginning of the 2024.
Standard 8VAC20-800-240-A
Based on observation of the back yard, the provider failed to ensure that the area and equipment of the family day home, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:

Observed in the back yard area, several broken toys and infant equipment.
Plan of Correction: The provider plans to obtain help to remove all items that are broken and not able to be used anymore.

Based on review of three family pets, the provider failed to ensure that documentation of the current rabies vaccination.

Evidence:
Family Pet # 2 did not have documentation of a current rabies vaccination.
Plan of Correction: The animal will be taken to the vet ASAP to ensure the pet is up to date with vaccines.

Based on review and interviews, the provider failed to ensure that a written attendance was taken daily.

Evidence:

The licensee stated that she was not taking written attendance of the children in care.
Plan of Correction: Provider will use the model form found on the DOE website and will be completed as the children arrive and depart.
