During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 06/26/2025 and completed on 06/26/2025. The on-site inspection began at 9:00 AM and ended at 10:15 AM. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 18. The inspector reviewed 3 children?s records, and 2 caregiver/household members records on-site. This inspection included document review, a tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There were no physical exams available for Child #2, and #3, who had been in care for over 60 days.
Plan of Correction: The caregiver notified the parent during the inspection. The parent will bring in a copy ASAP.
Standard 8VAC20-800-110-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the
Physician?s designee, or an official of a local health department.

The immunizations for Child #2, and #3, were not signed or dated.
Plan of Correction: The caregiver will remind the parents that a signed and dated copy of the immunizations.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children?s hands are washed with liquid soap and warm running water.

Child # 2 was fed a bottle, and the child?s hands were not washed.
Plan of Correction: The caregivers will make sure all children have their hands washed prior to eating
