During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the escription of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on November 13, 2024. The on-site inspection began at 12:30pm and ended at 3:13pm. The inspector reviewed compliance in the areas listed above. There were 36 children present and 6 staff. The inspector
reviewed 4 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are
documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Caregiver #5, who had been employed for two days, did not have a completed national criminal background check. Staff #5 was not working alone with children.
Plan of Correction: Staff will have not start until all required documents are received.
Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment.

Staff #1 and Staff #5, who had been employed for less than a week, did not have a central registry search requested prior to the first day of employment. Staff #1 and Staff #5 were not working alone with children.
Plan of Correction: I did not realize I could not submit it on their first day of employment. I will ensure all required documents are received before a new employee can start work.
Standard 8VAC20-780-450-A
Mats used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Child #5 and #6 did not have a bottom cover on the mat.
Plan of Correction: I will purchase a few cot sheets & blankets to have if a parent forgets to bring their child's blanket and sheet to the center. I will also contact the parents that morning to bring a blanket for their child.
