Inspector Notes

An unannounced monitoring inspection was conducted on 10/31/2022 from 11:03 am - 12:25 pm. There were ten children in care with 2 staff and 1 volunteer. The children ranged in age from 5 months - four years.
Children were observed during outdoor play, lunch and nap. Records were reviewed for three children, two staff and one volunteer.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to ensure that all applicants for employment, employees, applicants to serve as volunteers, and volunteers shall undergo a background check in accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter.

Evidence:
1. The results of a fingerprint background check for volunteer 1 were not available prior to beginning volunteering on 10/28/2022.
2. The program director confirmed that a fingerprint background check had not been obtained for this volunteer.
Plan of Correction: The director stated that volunteer 1 would make an appointment for a fingerprint background check today.
The director indicated that volunteer 1 had not been left alone with children and was supervised by a staff person with current background checks and training.

Based on interview, the center failed to ensure that an employee or volunteer of a licensed or registered child day program must not be employed or provide volunteer service until the center has the person's completed sworn statement or affirmation.

Evidence:
1. Volunteer 1, volunteer date 10/28/2022, lacks a completed sworn statement or affirmation.
2. The program director confirmed that a sworn statement or affirmation had not been completed by volunteer 1.
