Inspector Notes
An unannounced inspection was initiated on 04/09/2025 and completed on 05/19/2025 with local agencies in response to a complaint received by the licensing office on 04/08/2025 relating to allegations of non-compliance in the areas of programs and special care provisions and emergencies. The inspector reviewed 1 child's record and 1 staff record electronically on 04/09/2025 and 3 staff records on-site on 05/13/2025. This inspection included:

The preponderance of evidence gathered during the investigation supports the allegations; therefore the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/26/2025. A POC submitted after this date will not appear on the public website.
Violations
7The staff present on the afternoon of 04/02/2025 stated they did not feel they had sufficient English language skills to communicate with emergency responders.
1. Child 1 sustained a serious injury on 04/02/2025 at approximately 3:20 pm when her fingers were caught underneath a classroom door. Two of child 1's fingers were red and swollen and required first aid treatment from staff 2. Staff 2 and staff 3 stated the child was crying and indicated that her finger hurt.
2. Staff 2, staff 3 and staff 4 stated they did not contact the parent of child 1 because they lacked access to the parent contact information.
3. When the parent picked up child 1 at approximately 6:00 pm the child was crying, stated her finger hurt and "passed out in the car" according to the parent of child 1.
4. Child 1's finger was swollen, bruised and required medical attention.
(Pick up toys from the floor, keep toys clean and safe, get rid of broken toys, keep children cool outside and often offer drink of water).
The staff present on the afternoon of 04/02/2025 stated the program director was not present and they were not in charge of the administration of the center during this time. The staff present stated they were to contact the program director if they needed anything or if something happened.
We will also write a reminder in the weekly schedule.
The parent of child 1, age 3 years, was not notified immediately of a serious injury that occurred at approximately 3:20 pm on 4/2/2025. Staff stated they had no access to parent contact information for child 1. Staff notified the parent of the injury when the parent picked up child 1 at approximately 6:00 pm on 4/2/2025. The child was taken for emergency medication attention and diagnosed with a broken finger and cracked bone of a second finger.
1. Staff 1 confirmed that an injury report was not written for an injury sustained by child 1 on 03/26/2025. The parent of child 1 was notified of the injury by way of a text message.
2. The injury report for an injury sustained by child 1 on 04/02/2025 was written on 04/03/2025 and lacked several required elements.
3. Two injury reports for children in care, for prior injuries, lacked several of the required elements.
Director will make sure that all and any accidents will be immediately reported to parents and written reports will be signed by teacher and parent.
Attendance records were not available for 4/2/2025. Administrative staff stated that attendance records were not saved.
The center did not report to the department's representative an injury that occurred on 4/2/2025 while child 1, age 3 years, was in care. This injury required outside medical attention.
