Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 07/07/2025 and completed on 07/07/2025. The on-site inspection began at 10:05 am and ended at 1:05 pm. The inspector reviewed compliance in the areas listed above. There were 15 children present and 4 staff. The inspector reviewed 6 children's records and 8 staff records on-site on 07/07/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

5The inspection documents for the most recent inspection, conducted on 05/13/2025 and 05/19/2025 were not posted.
The TB screening for staff 1 was not updated every two years. The most recent screening was completed more than two years from the date of the original screening.
Staff 1 has not completed the Virginia Department of Education orientation course.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all of the required elements.
7 of 7 injury reports reviewed lacked documentation of any future action to prevent recurrence of the injury.
There was no written care plan for child 1 who has a diagnosed food allergy.
