Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 9/25/2025 and completed on 9/25/2025, as a part of the licensure period. The on-site inspection began at 2:40PM and ended at 3:54PM. The inspector reviewed compliance in the areas listed above. There were 16 children present and 2 staff. The inspector reviewed 5 children?s records on site on 9/25/2025 and 1 staff record electronically on 10/2/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, a six year old child was unsupervised for approximately 2 minutes.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #2, enrolled for 1 month, was missing the following required components: name, address, and phone number of two emergency contact?s; names of persons authorized to pickup the child; written agreements between the parent and the center.
