Inspector Notes

An unannounced monitoring inspection was conducted on 08/04/2022. There were 81 children present with 13 staff supervising. The physical plant, playground, evacuation drill logs, emergency preparedness plans, training log, children's records, staff records, attendance logs, medications and authorizations, menu's and the first aid kits were reviewed. The children were observed napping, being bottle fed, on the playground and working on individual Montessori materials. Areas of non-compliance were cited on the violation notice. Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in clean, safe and operable condition.

Evidence:
1. The refrigerator in the infant room recorded a temperature of 57 degrees F.
2. The ceiling on the second floor has cracked and the paints has peeled away.
Plan of Correction: 1. During the inspection the refrigerator temperature was turned down. The director will check temperature in two hours and will order a new thermometer and have service check on refrigerator if it does not cool down.

2. Ceiling repair has been reported to the owner and is on the list to be repaired.
Standard 8VAC20-780-290-A-3
Based on observation the center failed to ensure that electrical outlets shall have protective covers.

Evidence:
The outlet at the front desk did not have an outlet cover.
Plan of Correction: Outlet cover was replaced during inspection.
Standard 8VAC20-780-330-B
Based on observation the center failed to maintain required resilient surface under around and in the fall/use zones as required.

Evidence:
There is approximately one to three inches of resilient surface under around and into the fall/use zone.
Plan of Correction: Mulch and rubber mats have been ordered and will be installed on08/06/2022.
Standard 8VAC20-780-500-B
Based on observation, the center failed to ensure that a nonabsorbent surface shall be used for diapering.

Evidence:
In the Jr. Casa room there was a hole in the changing pad and therefore it was no longer nonabsorbent.
Plan of Correction: The Director stated that she would order a new changing pad today.
