Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documentation review, and interview.

A subsidy health and safety (SHSI) inspection was initiated on 7/28/21 and concluded on 8/2/21. There were eight children present with two staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview with the provider, caregivers who work directly with children did not complete the department's health and safety update course which was required by March 1, 2020. Evidence: The provider stated that the training had not been completed yet.
Plan of Correction: I will make sure we complete the training as soon as possible.
