Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 8/26/24. The on-site inspection began at 9:30 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above.There were 4 children present and 2 caregivers; the point total was 12. The inspector reviewed 4 children?s records and 3 caregiver/household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
8There was no information located or provided to show the date of the last National Fingerprint check for Person B.
Person A has been a household member for 5 months and there was no Sworn Disclosure statement on file.
Person A has been a household member for 5 months and there was no Fingerprint check.
Caregivers shall obtain a minimum of 16 clock hours of training annually.
Person B has obtained 11 of the 16 training hours for 2023 and Person C has obtained 4 of the 16 training hours for 2023.
Child A, 9 months of age was sleeping on a cot and not in a crib.
Two infants were present but there was only one crib set up in the home.
Child B's record did not contain emergency contact information, insurance and physician information.
Child C's record did not contain insurance policy number nor authorization from the parents to seek emergency care
1. The parents of child A, B and C were not provided in writing check in and out procedures, payment information, a number to leave a message, termination policies, hours and day s of operation. Child A has been enrolled for 7 months and child B has been enrolled for 4 days and child C has been enrolled for 5 months.
There was no documentation of shelter in place drills for 2023.
