During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 4/10/25 as a part of the provisional licensure period, The on-site inspection began at 2:50 pm and ended at 3:40 pm. The inspector reviewed compliance in the areas listed above. There were 2 children present and 2 caregivers; the point total was 4. The inspector reviewed 4 children?s records and 1 caregiver/household member(s) records on-stie.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation:
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical on file for child B, the physical was over due by 8 months.
Plan of Correction: I will get a copy of the physical.
Standard 8VAC20-800-110-B
Immunizations shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
The immunizations for child B did not have a signature by the physician or their designee. The signature was overdue by 8 months.
Plan of Correction: I will get a copy of the signed immunization records.

Repeat Violation:
Annually, the provider shall review with the parent the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
There was no annual update obtained for child A, the update was over 9 months overdue.
Plan of Correction: I will have parents review and sign off on the update.
