Inspector Notes

An unannounced, on-site monitoring inspection was initiated on August 19, 2025 and completed on August 25, 2025. The on-site inspection began at 11:04 am and ended at 11:50 am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregivers; the point total was 16. The inspector reviewed 2 children?s records and 1 caregiver/household member(s) records on-site on August 19, 2025.

This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on Sepetmber 1, 2025. A POC submitted after this date will not appear on the public website.

A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 was not completed and was overdue by two months.

Standard 8VAC20-800-60-B
Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record for Child #1 and Child #2 were missing second designated person to contact for emregency. Child #1 was missing proof of child's age and identity.
