Inspector Notes

An unannounced monitoring inspection was conducted July 14, 2023 from 12:15 pm to 12:55 pm There were 6 children for a total of 12 points directly supervised by 1 staff. The home, 1 staff record, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed resting. There was an assortment of age appropriate materials for children. The home was clean and organized.

Based on children records review, 2 out of 2 records, the provider did not obtain documentation of physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 and Child #2 did not have a physical examination in record available for review.
Plan of Correction: I will speak with parents to get physical for children.

Based on observation, the center did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.
Evidence:
1. The provider's license capacity is 5 children. On July 14, 2023, the provider had 6 children present at the family day home.
