Inspector Notes

An unannounced, on-site monitoring inspection was initiated on October 28, 2024 and completed on October 28, 2024. The on-site inspection began at 10:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregiver; the point total was 3. The inspector reviewed 2 children?s records and 1 caregiver/household member(s) records on-site on October 28, 2024. This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The Provider shall obtain documentation of a physical examination prior to the child's attendance or within 30 days of attendance.

Child A had no documentation of a physical examination. The child began attendance six months prior.
Plan of Correction: I will get the physical from the parent.
