Violations

1
Standard 22.1-289.036-A
Based on a review records and interview, the provider did not update central registry checks every five years for all household members.

Evidence:
1. The record for household member #2 contained a central registry dated 06/19/2017. The central registry should have been updated on or before 6/19/2022.
2. Provider acknowledged household member #2 central registry results were expired and she had not submitted paperwork for a new central registry.
Plan of Correction: Will mail out central registry background today.
