Inspector Notes

An unannounced monitoring inspection was conducted March 7, 2023 between 10:05am and 11:30am. There were 4 children present with 2 caregivers supervising within the supervision guidelines. The children were observed playing with age-appropriate toys. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programing, medication, fire drill log, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs. Standard 8VAC20-800-90-A
Based on record review, the provider did not obtain documentation of immunization before a child attends the family day home.
Evidence:
1. Child #4 started 11/7/22 and immunization record is dated 2/16/23.
2. Child #1started 10/1/722 and immunization record is dated 10/25//22.
Plan of Correction: I will make sure I have immunizations on the first day.
Standard 8VAC20-800-90-C
Based on record review, the family day home did not obtain documentation of additional immunizations every six (6) months for a child under the age of 2 years.
Evidence: Child #5 (DOB 2/19/22) record did not have additional immunization. The last update was 8/19/22.
Plan of Correction: I will tell the parents today I need an updated immunization record every 6 months.
