Inspector Notes

An unannounced monitoring inspection was conducted today between 10:35am and 11:30am. There were 6 children present with the Provider and 1 assistant for a total of 16 points within the supervision guidelines. The children were observed eating a morning snack, playing a phonics game and practicing drawing shapes. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 22.1-289.036-A
Based on record review and interview, every agent and household member did not undergo subsequent background checks every five years.
Evidence:
1. The providers most recent fingerprint background check is dated 4/16/2018.
2. Household member #1's most recent fingerprint background check is dated 4/16/2018.
3. Household member #1's most recent sworn disclosure statement is dated 4/13/2018.
4. The provider confirmed that the required background checks had not been renewed.
Plan of Correction: We'll have to make appointments and I'll send you the results once we get it finished.
UPDATE: Missing background check results were received 5/1/2023.
Standard 8VAC20-800-100-A
Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #4 (date of enrollment: 1/23/2023) record does not contain a physical exam.
Plan of Correction: I'll ask the parents to bring it to me.
UPDATE: The missing physical exam was obtained by 5/4/2023.
