Inspector Notes

An unannounced monitoring/renewal inspection was conducted today between 9:15am and 10am. There were 3 children present with the Provider and 1 assistant for a total of 11 points within the supervision guidelines. The children were observed drinking bottles and playing freely. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Repeat Violation
Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #2 (date of enrollment: 6/12/2023) record does not contain a physical exam.
Plan of Correction: I will send the parents a text right now and as for a copy within a week.

Based on record review, the provider did not obtain documentation that a child has been adequately immunized before attending the family day home.
Evidence: Child #2 (date of enrollment: 6/12/2023) record does not contain documentation of immunizations.
Plan of Correction: I will send the parents a text right now and as for a copy within a week.

Based on observation, menu requirements were not met.
Evidence: The providers food menu was not posted or placed in an area accessible to parents.
Plan of Correction: The menu was upstairs because we were working on it last night. Corrected during the inspection.
