Inspector Notes
An unannounced monitoring/renewal inspection was conducted today between 9:15am and 10am. There were 3 children present with the Provider and 1 assistant for a total of 11 points within the supervision guidelines. The children were observed drinking bottles and playing freely. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

3Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
