During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on October 22 2025 and completed on October 22 2025. The on-site inspection began at 11:00am and ended at 12:10am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 12. The inspector reviewed 2 children?s records and 3 caregiver/household member records.

This inspection included a tour of the home, a document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on October 31 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-60-B
A child's record shall contain the following information: Information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation.

Child # 1 record contained a food intolerance. Actions to take in an emergency situation were not obtained by the provider.
Plan of Correction: Actions to take in an emergency situation for food intolerance is noted in the child's chart.
Standard 8VAC20-800-900-A
To assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as uncut grapes shall not be served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking, i.e., grapes cut in small pieces.

The provider had uncut grapes available for the children to eat prior to lunch. After being notified that the grapes are a choking hazard, the provider removed the plate of grapes and put them onto the counter to cut before serving them.
Plan of Correction: Grapes will be sliced before offering to children.
