Inspector Notes
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
This inspection was initiated and concluded on 10/19/2021. The Provider was contacted by telephone and an inspection was conducted. There were 8 children present (21 points), ranging from 7 months to 3 years in age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 9 children records, 5 household records, and 3 staff records were reviewed.
Information gathered during the inspection determined that there were (2) non-compliances with applicable standards. These violations were documented on the Violation Notice issued to the program.

1. On the date of inspection (10/19/2021), the following immunization documentation was on file and older than 6 months: For Child #2 (dated - 02/23/2021), and for Child #4 (dated - 02/04/2021).
2. Based on documentation on file, both Child #2, and Child #4 were under 2 years of age.
Evidence -
1. On the date of inspection (10/19/2021), the following staff had CPS documentation on file that was older than five years: Staff #1 (CPS check dated - 04/28/2016), and Staff #3 (CPS check dated - 05/09/2016).
2. On the date of inspection, the following household members had CPS documentation on file that was older than five years: Household Member #1 (CPS check dated - 05/09/2016), and Household Member #2 (CPS check dated - 05/09/2016).
