Inspector Notes

An unannounced monitoring inspection was conducted on 1/18/23. The provider was present with 2 assistants and 7 children in care. Observed the children playing in centers, reading stories and playing with caregivers. Lots of interaction observed. The children had Prata, eggs and blueberries for breakfast. Based on the review of the children's, the provider failed to get proof of ID for two children in care.
Evidence: Based on the review of 5 children's files, Child 2 and Child 4 were missing proof of ID.
Plan of Correction: We have used model forms for most of the children, but do not have the forms for these two. I have reviewed the proof of ID but did not record it. We will get the information from the parents by tomorrow. We will also use the model forms so we have everything needed.
