Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a visual tour of the program. A monitoring inspection was initiated and concluded on July 29, 2019, There were 60 children present ranging in ages from 1 month to 6 years old with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of records, the center fail to provider proof of liability insurance.

Evidence: On July 20, 2021, the director did not produce documentation of liability insurance.
Plan of Correction: The Director will locate the documentation of liability insurance and insure that it is available for review.
Standard 22VAC40-185-60-A
Based on a review of children's records, the center failed to obtain and maintain required documentation for children's records.

Evidence:
Child #2, Child #3 and Child #4"s records do not contain documentation of persons authorized to pick up the child.
Plan of Correction: The Director will get the information from parents and place in children's records.
