A tuberculosis (TB) screening is required every two years from the last screening.

Staff #2 updated screening was received 1 month after it was due. Staff #4 updated screening was received 4 months after it was due. Staff #6 updated screening was received 5 months after it was due.
Plan of Correction: Reminders were set to request the Tuberculosis screening before they expire.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The playground equipment contained areas of cracking plastic on the spinning numbers, causing sharp edges accessible to children.
Plan of Correction: The playground equipment will be replaced.

Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually.

The information in the record of Child #6 and Child #9 had not been updated in over 17 months which is overdue by 5 months.
Plan of Correction: Parents were notified immediately about the missing paperwork and the updated forms were requested.

The center shall ensure that diaper ointments or creams are kept inaccessible to children.

Diaper creams and ointments were kept in an unlocked cabinet below the diapering area in Classroom #3 and Classroom #5, which were accessible to children. Diaper creams and ointments were kept on top of the counter in Classroom #7 and accessible to children.
Plan of Correction: A general reminder was given to all teachers that they must maintain diaper creams locked at all times.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.

Staff #2, who has been actively working at the center for the past 2 years, did not have any completed reference verifications on file.
Plan of Correction: Teachers were asked immediately to provide the 2 missing references.
