Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews. A virtual tour of the program was attempted, but due to technical issues failed.

A monitoring inspection was initiated and concluded on 08/17/2021. The Provider was contacted by telephone and an inspection was conducted. There were 9 children present (27 points), ranging from infant to preschool age, with 2 staff supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 9 children records, 2 household records, and 2 staff records were reviewed.

Information gathered during the inspection determined that there was a non-compliance with an applicable standard. This violation was documented on a Violation Notice issued to the program.

Based on review, 1 of 9 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (08/17/2021), a written authorization for emergency medical care (should an emergency occur and the parent cannot be located) was not available for review for Child #2.
2. A written objection to provision of medical treatment on religious or other grounds was not stipulated.
Plan of Correction: The Parent of Child #2 shall be informed of this requirement and the authorization or written objection needed to be on file.
