Inspector Notes

An unannounced monitoring inspection was conducted on 4/25/2023 from 9:30 am-10:40 am. There were 7 children (18 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed participating in circle time. Areas of non-compliance are identified in the Violation Notice. Based on record review and interview, one of five records did not have an annual review of emergency contact information with the parent to confirm the information is correct and to obtain the parent's written acknowledgment of the review.

Evidence: Child #2's last annual emergency contact review was on 12/6/2021. The start date for child #2 is 12/7/2021.
Plan of Correction: I'll have the parents review and sign the form at pick-up today.

Based on record review and interview, proof of identity was not obtained within 7 days.

Evidence: Child #2 does not have signed documentation of viewing the proof of identity for the child. The start date for child #2 is 12/7/2021.
