Inspector Notes

An unannounced monitoring inspection was conducted today. There are no children present during the inspection.
3 child and 4 Caregiver/Household records were reviewed. See Violation Notice for non-compliances cited.
The license posted is not the most currently issued license. A current copy was found during the inspection.
Emergency evacuation drills, emergency supplies, first aid supplies, first aid/CPR certification, fridge/freezer temperatures and vehicle inspection are all in compliance.

Based on observation, the most recently issued license was not posted.
Evidence:
The license posted expired in 2021.
Plan of Correction: Posted correct license as soon as possible.
Standard 22.1-289.036-A
Based on a review of records, one caregiver and two household members did not have documentation of a repeated nationwide fingerprint search within the allowable 5 year time frame.
Evidence:
1. Caregiver A's fingerprint results on file were dated 2/13/18. (expired)
2. Household member #1's fingerprint search results on file were dated 2/13/18. (expired)
3. Household member #2's fingerprint search results on file were dated 2/27/18. (expired)
Plan of Correction: I already had made an appointment at the tine of the visit. The appointment was for 6/27/23
Standard 22.1-289.036-B-4
Based on a review of records, one household member's record did not contain documentation of a nationwide fingerprint search within 30 days of turning 18 years old.
Evidence:
1.HHM #3 turned 18 years old on 10/27/22. Fingerprint results were dated more than 30 days later.
2. Household member (HHM)#3's record contained documentation of a fingerprint search result dated 12/27/22. While now on file, the results were obtained more than 30 days after turning 18 years old.
Plan of Correction: This violation was corrected on 12/27/2022.
Standard 8VAC20-800-170-A
Based on a review of records, one Household Member (HHM) record did not contain documentation of a negative Tuberculosis Screening result, within 30 days of turning 18 years of age.
Evidence:
1. HHM #3 turned 18 years old on 10/27/2022.
2. There is no TB screening result available for HHM#3.
Plan of Correction: I already scheduled an appointment for TB testing on 6/27/23.

Based on a review of records, caregiver and household member records did not contain documentation of a subsequent TB screening result dated within 2 years for the previous screening results.
Evidence:
1. Caregiver A's TB result on file is dated 5/30/21. (expired)
2. HHM #1's TB result on file is dated 5/30/21. (expired)
Plan of Correction: Had appointments on 6/27/23.

Based on a review of records, one caregiver record did not contain documentation of the completion of 16 hours of annual training.
Evidence:
Caregiver A's record contained documentation of 8.5 of the 16 required annual training hours for 2022.
