During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on December 4, 2024 and completed on December 4, 2024, as a part of the licensure period. The on-site inspection began at 10:10am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 38 children present and ten staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review and a tour of the facility,

Annual approval from the health department shall be provided for meeting requirements for water supply, sewage disposal system, and food service, if applicable.

Provider has not completed the required annual health inspection, after the first license.
Plan of Correction: Will make an appointment with the health department to schedule the annual inspection.

In areas used by preschool age or younger children, electrical outlets shall have protective covers.

Children in the preschool primary room were working on Montessori activities near six uncovered outlets that were within their reach.
Plan of Correction: The six outlets were covered during the inspection.

REPEAT VIOLATION
Prescription medication may be administered if the parent has completed a written authorization from a parent or guardian.

There was no written parent/guardian authorization to administer prescription medication to child #5.
Plan of Correction: Will contact parents to complete the authorization form.

When food is brought from home the food container must be clearly dated and labeled in a way that identifies the owner.

Three food containers in the toddler room were not dated or labeled with the child's name.
Plan of Correction: Will make sure to label and date all meal containers brought from home going forward.
