Inspector Notes

A monitoring inspection was initiated on 5/2/2023 and concluded on 5/2/2023. There were a total of 9 children (26 points) observed with 2 caregivers present. The children were engaged in the following: toy play, songs, Legos, handwashing, diapering and snacks. The home, 3 child records, 4 caregiver/household member records, evacuation drills, emergency supplies and attendance were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, backgrounds checks are not completed every 5 years from the last search.
Evidence: The following have not updated their fingerprint results every five years.
Caregiver #1 fingerprints are dated 2/20/2018
Household #1 and #2 fingerprints are dated 2/20/2018
Plan of Correction: The caregiver and household members will schedule a fingerprint appointment. Once the results are received they will be filed in the records and forwarded to the Inspector.
Standard 8VAC20-800-280
Based on observation, cleaning agents were observed in reach of children.
Evidence: Cleaning agents were found in an unlocked cabinet under the sink in the bathroom used by the children.
Plan of Correction: A lock will be placed in the cabinet or the cleaning agents will be stored away from the children.
Standard 8VAC20-800-60-B
Based on a review of children records, children records did not contain documentation of the Parent Information form.
Evidence: Child #1, first day of attendance 3/8/2023 does not have documentation of the Information for Parents form in the record.
Child #2, first day of attendance 8/23/2021 does not have documentation of the Information for Parents form in the record.
Plan of Correction: The provider will have the parents sign the Information for Parents form and file in the children's records.
