During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/3/2026. The on-site inspection began at 9:45 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 caregivers and 1 volunteer ; the point total was 23. The inspector reviewed 5 children?s records and 3 caregiver/household members records.
This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Standard 8VAC20-790-250-F
Caregivers who work directly with children shall annually attend the department's health and safety update course.
Caregiver #1 did not have an annual health and safety training, The annual update is 20 months past due.
Plan of Correction: The caregiver will take the training and print out the certification to be filed in the record.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
There was no physical exam records available for Child #4, who had been in care for over 90 days.
Plan of Correction: The parent will bring in the current physical examination.
