Inspector Notes

An unannounced monitoring inspection was conducted from3:10 PM--4:20pm. At the time of entrance, 7 children (age range 5-10) were in care with3 staff members present. The sample size consisted of 5 children?s records and 3 staff records. Children were observed playing outside and working on art activities. Medication administration was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard 8VAC20-780-510-B
Based on medication review, documentation review and staff interviews, the licensee did not ensure that prescription medication shall be given to a child only with written authorization from the parent.
Evidence: The medication log for Child #5 was reviewed and medication (methylphenidate) had been administered a total of 13 days between June 15, 2023, and July 7, 2023. The medication authorization for Child #5 was reviewed and the parent did not indicate the name of the childcare provider authorized to administer medication.

Standard 8VAC20-780-560-F
Based on observation and staff interviews, the licensee did not ensure that when centers choose to provide meals or snacks, they shall have a menu listing the food to be served for meals and snacks during the current one-week period.
Evidence: The menu for the current week did not include all the food items that were to be served current week. It only listed an entree (corndog) and milk for the lunch served on July 10, 2023. Center Director confirmed children were served corn, 100% fruit juice and goldfish crackers, as a part of the lunch.
