Inspector Notes
An unannounced monitoring inspection was conducted from3:10 PM--4:20pm. At the time of entrance, 7 children (age range 5-10) were in care with3 staff members present. The sample size consisted of 5 children?s records and 3 staff records. Children were observed playing outside and working on art activities. Medication administration was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program
Violations
2Evidence: The medication log for Child #5 was reviewed and medication (methylphenidate) had been administered a total of 13 days between June 15, 2023, and July 7, 2023. The medication authorization for Child #5 was reviewed and the parent did not indicate the name of the childcare provider authorized to administer medication.
Evidence: The menu for the current week did not include all the food items that were to be served current week. It only listed an entree (corndog) and milk for the lunch served on July 10, 2023. Center Director confirmed children were served corn, 100% fruit juice and goldfish crackers, as a part of the lunch.
