Inspector Notes

An unannounced inspection was initiated on 07/05/2024 and completed on 07/31/2024 in response to a complaint received by the licensing office on 07/05/2024 relating to allegations of staffing training and qualifications, staffing and supervision and special services. The inspector reviewed 3 staff records electronically . This inspection included document review, interviews and observations .

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/07/2024. A POC submitted after this date will not appear on the public website.

Based on interview and documentation, the licensee did not ensure during the stated of hours of operation there shall be on the premises one or more children are present with one staff member and available member volunteer, staff or other employee with direct means for communication between the two of them.

Evidence:
On June 5, 2024, staff #1 was alone with 2 children on the premises . Confirmed by staff #1 , staff #2 and staff #3.

Based on record review and staff interview, the licensee did not ensure at least two staff who have CPR and First Aid were on premises during center hours of operation and whereever children are in care.

Evidence:
On July 5, 2024, there was only one staff on premises with CPR and First Aid until 8:10am. Confirmed by staff.
