Inspector Notes

An unannounced code compliance inspection was conducted on 6/14/2023 from 9:00 AM to 10:15 AM. There were 29 children present, ranging in age from two to five years, with 8 supervising staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of 5 children?s records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Based on records review and staff interview, the facility did not obtain results of an out-of-state central registry and criminal history search for an employee who has resided outside of Virginia within the last 5 years.

Evidence:
1. Staff # 3's (DOH: 8/2021) sworn statement form dated 6/16/2022 indicated that they resided outside of Virginia within the last 5 years.

2. There was no documentation of the out-of-state criminal record check and central registry results for Staff #3 from a state in which they had listed as residing within the previous five years.
Plan of Correction: I will submit the clearance for the out-of-state background checks by 6/16/2023.
