Inspector Notes

An unannounced inspection was completed on 07/15/2022 from 09:53 A.M.-12:30 P.M. At the time of the inspection fourteen children were in care with six staff and one administrator present. Two children records, four staff records, four injury reports, emergency plan, first aid kits, emergency supplies, water temperature, evacuation drills, playground, and posted required information were reviewed. Children were observed during small group activities which included fine motor activities, outside play time, handwashing, bathroom procedures, lunchtime, and naptime.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on a record review of children's files, the center failed to obtain documentation of immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. Child B (date of enrollment: 05/09/2022) did not have documentation of an immunization record on file.

Based on record review of children's files, the center failed to obtain documentation of a physical examination within thirty days after the first day of attendance.

Evidence:
1. Child B (date of enrollment: 05/09/2022) did not have documentation of a physical examination on file.

Based on review of documentation and staff interview, the center did not ensure that a monthly practice evacuation drill was conducted.

Evidence:
1. The documentation of the evacuation drills did not have dates written for monthly drills in May or June 2022.
2. Staff #2 stated the last fire drill was conducted at the end of May 2022 but they did not have one in June 2022.
