During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/24/2025 and completed on 07/29/2025. The on-site inspection began at 9:30 AM and ended at 11:30 PM. The inspector reviewed compliance in the areas listed above. There were 19 children present and 6 staff. The inspector reviewed 5 children?s records, 5 staff on-site. This inspection included document review, a tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children.

In the infant classroom, the cleaning solution and sanitizing solution were in an unlocked cabinet.
Plan of Correction: The director will install new locks and will make sure they are preventing entry to cabinet.
Standard 8VAC20-780-320-B
The restroom shall be equipped with soap, toilet paper, and disposable towels or an air dryer within reach of children.

The restroom used by the children did not have disposable towels or an air dryer. There was a bin of clean wash clothes.
Plan of Correction: Paper towels will be placed in the restroom and will be monitored and replaced as needed.

Documentation of at least 2 references to character, reputation, and competencies are to be checked prior to employment.

Staff #2, who has been actively working at the center for the past month, did not have any completed reference verifications on file.

Staff #3, who has been actively working at the center for the past 4 months, did not have any completed reference verifications on file.
Plan of Correction: References will be obtained and will be completed prior to hire.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were 6 children present according to the attendance record for the week, and on Monday, Tuesday and Wednesday none of the children were marked as they departed.
Plan of Correction: Attendance will be written and monitored by director.
