Inspector Notes

This inspection was conducted by licensing staff using and alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was conducted on January 28, 2022. There were 5 children present directly supervised by 2 staff. The children were in circle time at the table.

Limited monitoring of child care requirements occurred during today?s visit. Licensing inspector monitored supervision of children, physical plant, staff/child ratios, adequate and approved space, programming, and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff records review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff #1 Tuberculosis Screening expired on 7/08/2020.
2. Staff #2 Tuberculosis Screening expired on 4/24/2020.
