Inspector Notes

A renewal inspection was initiated and concluded on2/1/2023, approximately 10:35 am to 11:15 am. There were a total of 5 children, totaling 13 points, with 2 staff members present. Children were observed in indoor play. Staff were actively engaged and interactive with the children.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 2 staff records, and 1 household record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on household records review, the Aathorize the family day home, did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Household member moved in the home January 12, 2023.
2. Provider did not have documentation of submitting central registry to obtain results for household members during.
Plan of Correction: The household member has been removed from the home.
