Inspector Notes

An unannounced, on-site monitoring inspection was initiated on July 17, 2024, and completed on July 23, 2024. The on-site inspection began at 10:08 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 5 children present, totaling 18 points and two staff. The inspector reviewed two children?s records and two staff records on-site on July 17, 2024. This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 30, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-180-A
Based on documentation review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years.
Evidence:
1. Staff 2 tuberculosis (TB) tests or screening expired on February 2, 2024.
Plan of Correction: My daughter, my assistant and I have gotten our TB test done last week. In the future I will check expiration datesin advance.
Standard 8VAC20-800-570-A
Based on observation the provider failed to ensure that a caregiver does not exceed 16 points by using the point system to determine if an additional caregiver is needed.
Evidence:
1. Staff 2 was providing care for 5 children, totaling 18 points.
2. Licensing was at the home for 30 minutes before Provider arrived at the family day home.
Plan of Correction: One of the children were supposed to come one hour later so I could step out and run an errand. The next time I iwll have the parent to wait with the child until I return.
