During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/16/2025. The on-site inspection began at 11:30 AM and ended at 12:20 PM. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 11. The inspector reviewed 3 children?s records and 4 caregiver/household member records. This inspection included document review, a tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Child #2 (date of enrollment: 8/20/2024) and child #3 (date of enrollment: 8/20/2024) records do not contain documentation of a physical exam.
Plan of Correction: The parents were contacted during the inspection and an updated record will be requested. I will have the document by Monday.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations
for a child who is not exempt from the immunization requirements one every six months for children under the age of two years.

Child #2 (14 months) most recent immunization record is dated 5/21/2024.
Plan of Correction: The parents were contacted during the inspection and an updated record will be requested. I will have the document by Monday.
