Inspector Notes

An unannounced monitoring inspection was conducted on 6/1/22 from 9:30 - 11:15amm. At the time of the inspection there were 12 children in care, ages one year old to four years old (27 points), with three caregivers present. Records were reviewed for five children and five caregivers. Children were observed participating in various activities. There was no medication at the family day home currently. Emergency plan and emergency supplies were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed with the provider during the exit interview.

Based on a review of five child records, it was determined that the licensee did not ensure that the record for each child contains all of the required information.

Evidence:
1. The record for child #1 did not indicate whether or no the child previously attended child care.
2. The record for child #3 did not indicate whether or no the child previously attended child care.
3. The record for child #5 did not indicate whether or no the child previously attended child care.
4. Caregiver #1 confirmed that the records for child #1, child #3 and child #5 did not contain all of the required items.
Plan of Correction: The provider responded: The parents of each child will be asked to provide the missing information. All child records will be reviewed on a regular basis to ensure they are complete.
