Inspector Notes

An unannounced monitoring inspection was conducted on 12/6/2023 from 11:15am to 1:15pm. At the time of the inspection there were 9 children in care totaling 18 points and 3 caregivers, including the provider. Children were observed eating lunch and preparing for nap time. Emergency supplies, evacuation drills, and records for caregivers, children and household members were reviewed. The outside play space was also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on record review and interviews, it was determined that the provider did not ensure that all employees undergo a background check prior to employment and every five years thereafter.
Evidence:
1.The records for staff #3 and staff #4 did not contain the required fingerprint-based criminal history record search and determination letter for renewal searches.
2. The staff records contained criminal background searches dated 11/3/2023, using an incorrect search process.
3. The provider confirmed the renewal criminal history searches were completed with the wrong search process.
Plan of Correction: Both staff members will schedule with Fieldprint to have the renewal fingerprint- based background checks completed immediately.
