Inspector Notes

An Initial Inspection was conducted this day. Provider is currently licensed at another location and plants to move to a new location beginning 12/13/2021. No children were present at the time of the inspection. Walk through of all 3 levels of the home was completed. Observation of the front and back yards was conducted. Compliance with several regulations were not determined since the provider was not operational at this location.
